I went to a periodontist on Wednesday (the only appointment slot available for months (someone had cancelled)).
She had a digital x-ray machine, something I hadn't seen before - a bit more awkward to bite than film, but almost instant viewing on screen, and uses a lower x-ray dose. After the usual comments that my teeth are a bit strange, she thought with some training from the hygenist, an estimated four sessions of two hours each at £300/hr cleaning the roots, and some magic pig extract, I can probably keep the molars for another twenty years, but don't count on it. (All the molars are affected, all the other teeth are okay.)
The whole "cut open the gums to clean the teeth roots" thing is a bit scary, but so is "leave them as they are and you'll get repeated deep infections until they fall out" (and so is "we could take them out and put implants in").
Next appointment is in July, unless I get another cancellation (it's only a few minutes cycle from work - they seem to have messed up their website at the moment).
